This form seeks your consent for audio recording of therapy sessions and the use of AI tools to assist with clinical note-taking. Please read carefully and ask any questions before signing.
This consent is optional. You may decline without any impact on your therapy. If you choose not to consent, your sessions will continue as normal without recording and I will write my notes in the usual way.
What This Involves
To provide the best possible care, I would like to audio record our sessions. A transcript of the recording lets me use AI tools to draft accurate session summaries rather than relying on memory alone. This supports better note-keeping and, in turn, better care. AI assists me with writing notes only. It never makes decisions about your therapy.
How It Works
- Recording: the session is recorded on my phone using an app that transcribes speech on the phone itself. The audio is not uploaded to any cloud service.
- Audio deleted: the audio recording is deleted within 24 hours. No audio recording is ever stored as part of your clinical record.
- Transfer: the transcript is sent from my phone to my computer over an encrypted local connection between the two devices. It does not pass through the internet.
- De-identification: an AI model running entirely on my own computer removes names, places and other identifying details from the transcript. The identifiable transcript never leaves my own devices.
- Summary: only the de-identified transcript is sent to Claude, a cloud AI service provided by Anthropic and hosted outside Australia, to draft a session summary. Nothing that identifies you is sent, and the text is handled under Anthropic's privacy policy.
- My review: I read, correct and approve every summary before it goes into your clinical record in Halaxy. I remain fully responsible for the content of your record.
The summary is used solely to support clinical note-taking for your record and for my professional supervision, where your identity is always protected.
Limitations You Should Know About
- Speech-to-text and AI summaries can contain errors, misattribute who said what, or miss nuance. My review step exists to catch these, but no process is perfect.
- De-identification greatly reduces, but cannot entirely remove, the chance that something distinctive you said could be recognisable to someone who knows you.
- AI tools can reflect biases in how they summarise. I treat every summary as a draft to be checked against my own recollection of the session, not as a record in itself.
- If any part of this process fails or a tool becomes unavailable, I write my notes in the usual way. If I ever became aware that your information had been exposed, I would tell you and act in line with my Privacy Policy.
Your Rights
- You may withdraw this consent at any time by informing me verbally or in writing
- Withdrawing consent will not affect your therapy or our therapeutic relationship in any way
- You may request that recording not take place for any particular session, or ask me to stop recording partway through a session
- You may ask me at any time how the process works, or to see the summary that went into your record
- If you have any concerns at any point, I encourage you to raise them with me directly. You can also raise a concern with PACFA or the Queensland Office of the Health Ombudsman, as set out in my Informed Consent form
